Nursing and Residential Triage

The Nursing and Residential Triage Tool is designed for both health care and non-health care professionals to assist in safely determining the most appropriate priority and referral pathway for your clients.

It aims to reduce unnecessary Emergency Department attendances by ensuring that lower acuity conditions are dealt with by the most appropriate health care provider.

Training will be provided to key members of your home to enable you to cascade the tool to the rest of your staff. All training materials are provided as well as online

resources to ensure understanding and to track progress.
